Gynecomastia surgery in Iran: all you need to know

We at AriaMedTour receive a large number of requests every month from people from different countries asking for a male breast reduction or gynecomastia surgery in Iran. Since gynecomastia surgery is a safe procedure and you can get it at a decent price in Iran, men from other countries are increasingly showing interest in traveling to here to undergo the procedure.

In this article, we are going to both shed light on gynecomastia surgery and provide you with some information about this treatment in Iran.

What is gynecomastia?

Gynecomastia is a common condition in men that causes the breast tissues to swell and become larger than normal. Although gynecomastia is benign, it often leads to psychological problems and is a source of embarrassment for individuals, making them prone to teasing and name-calling.

Why does gynecomastia happen?

Abnormal enlargement of breast tissue in men, can be caused by several factors, including a hormonal imbalance (low testosterone and high estrogen levels), or certain medications such as spironolactone (Aldactone), calcium channel blockers (used to treat hypertension), anti-androgens, anti-ulcer drugs, as well as drugs of abuse such as alcohol, marijuana, heroin, amphetamines, and methadone.

Some medical conditions that might be associated with gynecomastia are:

  • Cirrhosis of the liver
  • Chronic kidney failure
  • Testicular cancer
  • Disorders of the testes (male sex organs) including infection, trauma, or inborn disorders
  • Hyperthyroidism
  • Anti-androgen treatments for prostate cancer

Types of gynecomastia

According to a classification proposed by the American Society of Plastic Surgeons, gynecomastia is categorized into four stages (depending on the size of the breasts and severity of the problem).

  • Grade I: Small breast enlargement with localized button of tissue around the areola
  • Grade II: Moderate breast enlargement exceeding areola boundaries with edges that are indistinct from the chest
  • Grade III: Moderate breast enlargement exceeding areola boundaries with edges that are distinct from the chest with skin redundancy
  • Grade IV: Marked breast enlargement with skin redundancy and feminization of the breast
Gynecomastia grades (types)

Grades of gynecomastia according to the classification of the American Society of Plastic Surgeons

Gynecomastia diagnosis and treatment

We recommend that you consult a specialist to make sure that the enlargement of your breasts results from gynecomastia not a threatening disease like breast cancer. Your doctor will ask you a number of questions about your and your family’s medical history and medications you take.

The doctor will also do a physical examination and may ask you to undergo a blood test and mammography in order to make a gynecomastia diagnosis. Depending on the results of the initial tests, you may need to further undergo CT scans, MRI scans, testicular ultrasounds, and tissue biopsies.

After finding the root cause of gynecomastia, the first step is to treat the condition that has caused gynecomastia. If a certain medication you are taking is the cause, your doctor may ask you to stop it or substitute it with another medication.

Some medications used for breast cancer and other conditions, including raloxifene (Evista), tamoxifen (Soltamox), and aromatase inhibitors (Arimidex) can help in reducing gynecomastia, but they are not specifically approved for this use. There are medication-based therapies like those prescribed to alter the hormone levels, but their efficacy is not 100 percent and they often have unwanted side effects

Most cases of gynecomastia improve on their own over time without any treatment, and this may take one year or two. If this doesn’t happen, treatment becomes necessary. The fastest and most effective gynecomastia treatment option is surgery.

How is male breast reduction surgery done?

Depending on whether the bulging of breasts is caused by excessive accumulation of fat in breasts, which is called ‘false gynecomastia’, (also, pseudogynecomastia, lipomastia or adipomastia) or excessive growth of breast tissue (gynecomastia), two techniques are used: liposuction and mastectomy.

Liposuction

If your doctor recommends you to have a liposuction procedure for your enlarged breasts, make sure to choose a qualified plastic surgeon who has enough experience in liposuction of male breasts. Liposuction for male breasts can be performed under local anesthesia or general anesthesia, depending on your preferences.

Mastectomy (excision technique)

Mastectomy involves the removal of excess breast tissues or skin. Excision technique for gynecomastia is often done endoscopically with a small incision.

Combination of liposuction and excision technique

Sometimes the gynecomastia patient needs to both get the fat in the breast area removed and the excess tissue removed. In this case, both liposuction and excision technique are used to get the ideal result from this cosmetic surgery.

Most doctors first remove fat deposits lying around the areolas and under the arms, then by making a small incision under the areola, cut out the dense, rubbery tissue that cannot be suctioned out through with the cannula.

Am I a candidate for gynecomastia surgery?

You can consider a male breast reduction surgery if:

  • You have good overall health and don’t suffer from a life-threatening illness or a condition that impedes healing.
  • Your enlarged breasts haven’t got better after 1-2 years and they cannot be corrected through other treatments.
  • You are a non-smoker and non-drug user
  • You have a positive outlook and have realistic expectations.
  • You are physically healthy and of relatively normal weight. If you are overweight or obese, it is recommended that you consider a weight loss program first, because as you lose weight, your breasts too get smaller naturally.
  • Your breast size has stabilized
  • You are feeling bothered and embarrassed about your female-like breasts, and has lost your self-confidence

Male breast reduction risks and side effects

Any surgery has some potential risks and complications. Be as informed as possible about the benefits and risks of gynecomastia surgery. The complications that might occur after surgery are discussed prior to your surgery.

Gynecomastia surgery risks are reactions to anesthesia, tape, suture, glues, topical preparations or injected agents, breast asymmetry or irregularity, bleeding, change in nipple sensation, fat necrosis, fluid accumulation, infection, poor healing, pain, scarring, damage to nerves or vessels, and deep vein thrombosis, cardiac and pulmonary complications.

Most of these complications are rare and temporary if occur. Choosing the right doctor and hospital are key in reducing the chance of developing complications.

How is the gynecomastia surgery recovery like?

The male breast reduction procedure isn’t a major surgery. After the completion of the surgery, bandages and dressings will be applied to the incisions and you will receive post-op care before being discharged from hospital. You may be asked to wear a support garment or elastic bandage to reduce swelling and support the new chest contour as it heals.

Special instructions are given by your surgeon as to how to care for the surgical site and what medications to use, possible risks and complications of gynecomastia surgery, and dos and don’ts for the recovery period.

Make sure to discuss your specific concerns with your surgeon before surgery, such when you can resume your normal activities, when to come back for follow-up, potential risks, and complications, and what are post-op care for gynecomastia surgery.

It is very important to strictly follow your physician’s instructions in order to get the most satisfactory result from your treatment.

Our medical consultants at AriaMedTour are always prepared to answer all your questions both before and after surgery. We stand beside during the time you are staying in Iran for your gynecomastia surgery and continue our online follow-up even when you fly back home.

Since sedative and anesthesia is used, you won’t feel any pain while the surgery is being done, but after surgery, you might feel some pain in your chest, which can be relieved by taking pain killer medications.

You can resume your routine work within a few days, but you should avoid strenuous activities for some 3 weeks.

Enlarged nipples during puberty are a common concern, while in many cases it’s resolved on its own, it’s understandable that they’re impacting your self-esteem if still unresolved at 24.
Treatment options depend on the underlying reason which could be better identified through a medical examination. Usually maintaining a healthy weight, a balanced diet, and regular exercise are among the primary non-surgical methods, however, they might only work for some, and results can be subtle. In some cases, hormone-based medications or a change in the medicines you’re already using may be prescribed. If you’re looking for a more definitive solution with more dramatic results, gynecomastia surgery may be the best treatment option.
Still, make sure you consult a board-certified plastic surgeon experienced in gynecomastia to discuss your concerns. They can perform a physical examination and discuss all possibilities, including non-surgical and surgical options.

I am young and young young young very active and noticed some breast tissue development a few months ago. I’m worried surgery will affect my ability to lift weights and play sports. Did any of your patients experience limitations after surgery? For how long?

While there will be some limitations in the initial recovery period, getting back to your regular workout routine is definitely possible! You’ll just need some time.
The initial recovery period typically involves 1-2 weeks of rest, with light walking and gentle stretching encouraged. It’s followed by a gradual return to exercise over 4-6 weeks. This means focusing on cardio and lower body exercises initially. Upper body exercises, including weightlifting, can come back around week 6-8.
Remember, this is a general guideline, and each individual is different. So listening to your body and consulting your doctor is the key. Feeling pain and discomfort in your chest area while exercising is a sign to slow down or stop. You’ll probably need more time. Don’t push yourself too hard. Progression should be gradual, with weight and intensity increasing step by step to minimize the risk of injury.
And more importantly, get your doctor’s approval before diving back into intense workouts. With a little patience and care, you’ll be back to weightlifting and your other sports activities in no time!

I’m a 15-year-old boy with large breasts. It’s horrible but my parents are hesitant about surgery at my age. I’ve had these puffy nipples for more than a year now. But how long should I wait? I’ve read about gynecomastia during puberty but please tell me the timeline. When can I have the surgery? Will it come back after surgery?

Thanks a lot for your question. It’s understandable to feel self-conscious about gynecomastia, especially at your age. However, you should know that gynecomastia is quite common during puberty, affecting up to 50% of boys between 11 and 15 years old so you’re not alone. In most cases, it resolves on its own within a few months to a few years as your hormone levels stabilize. Therefore, there’s a good chance you won’t need the surgery. It’s usually recommended that you wait until puberty is complete (around 18 years old) before considering surgery for gynecomastia. This allows time for the tissue to potentially shrink naturally. If the issue persists after puberty, you can consider the possibility of surgery with your doctor at that time. Meanwhile, you can see a doctor. They can confirm if it’s gynecomastia and reassure you and your parents about the timeline and potential for natural resolution.
Recurrence is rare. Gynecomastia surgery offers lasting results as it removes excess tissue and fat. There’s a small chance of the condition’s recurrence due to factors like incomplete removal during the surgery, unhealthy lifestyle, medication, and some diseases.

I am considering gynecomastia surgery but worried about scarring. I mean i really don’t want to solve one problem and fall into another one. My self esteem is already ruined due to these large breasts. How noticeable is scarring after gynecomastia surgery? Are there ways to minimize it?!

I understand your concern. While some scarring is unavoidable, advancements in technique and proper aftercare can significantly minimize their visibility. Experienced surgeons strategically place incisions in areas easily hidden by clothing or natural chest contours. Following post-operative instructions like proper wound care and wearing compression garments is crucial for optimal healing and minimizing scar formation. Scars typically fade over time, becoming less noticeable within a year.
Remember, many patients report a significant boost in self-esteem after surgery, with the positive body image outweighing any concerns about scars. If minimizing scars is a top priority, discuss your concerns with your doctor during a consultation. They can explain their specific techniques and show you examples of their patients’ results to help you feel confident about your decision.

Yes, and thanks a lot for your question. Actually, it’s great that you’re considering gynecomastia surgery. In fact, while gynecomastia is common during puberty, it’s also a common issue for men above 50. So it’s not strange to opt for the surgery at this age. Many men in their 50s and beyond successfully undergo gynecomastia surgery. The focus is on your overall health, not just your age. It is true that background health conditions can make the surgery riskier. However, if your blood pressure is under control, it won’t be a major issue for surgery.
Still, an in-person consultation with a specialized surgeon is crucial. They will assess your health, including blood pressure control, to determine if surgery is safe for you. Today, surgeons use minimally invasive techniques and also discuss anesthesia options to minimize the surgery risks for special cases. I really hope you’ll get the result you desire through a safe and successful surgery.

How long will i need to wear compression garments after gynecomastia surgery? Is it a fixed thing or can i hope for a shorter period if i heal quickly? But pessimistically what is the longest time one had to wear the compression garment? Are they comfortable and won’t be noticeable under my clothes?

It’s important to wear compression garments after some surgeries including gynecomastia as it helps reduce swelling and discomfort. However, There’s no set timing both due to each person’s individual healing process and also your surgeon’s approach.
Considering the surgery techniques and the surgeon’s attitude, some surgeons believe wearing compression garments should only be advised to manage the initial swelling and bruising. So they recommend wearing them for 1-2 weeks only, followed by selective use of a compression undershirt until you’re fully comfortable not wearing one.
Other surgeons believe compression garments must be worn for longer to address the persisting swelling after the initial recovery and provide support when doing exercises. Accordingly, they usually recommend wearing the garment for about six weeks.
So the longest someone typically wears a garment is around 6 weeks, still consider some variations for your case.
Make sure you discuss your concerns about wearing a compression garment with your surgeon before the surgery. They will also monitor your recovery and advise you about the best timing considering your specific situation.
Anyway, compression garments are designed to be fit under clothing but comfortable at the same time. However, comfort can be subjective. Some people find compression to be very supportive and helpful, while others might find it restrictive.
I really hope I answered your questions and wish you the best of luck with your upcoming surgery.

Can doing exercises like push-ups or weightlifting actually get rid of gynecomastia? I’m 17 and thought maybe working out at a young age would help.

Unfortunately, chest exercises won’t directly get rid of gynecomastia.
Gynecomastia involves an overgrowth of glandular or fatty tissue in the chest area. Exercise primarily targets and builds muscle, not glandular or fatty tissue. Regarding Gynecomastia, exercise can only be beneficial in maintaining a healthy weight. And in cases where gynecomastia is caused by excess weight, weight loss might lead to an improvement in breast tissue appearance.
At 17, your body is still developing. In some cases, gynecomastia can resolve on its own during puberty. However, if it’s causing you concern, it’s best to consult a doctor. They can properly diagnose the cause and recommend the best course of action. This might involve monitoring the situation, medication in some cases, or potentially considering surgery as you get older (usually 18 or over).
