Rhinoplasty in Iran: all you need to know

Iran is known as the ‘nose job capital of the world’, with more per capita procedures than any other country (seven times more than the United States). With well-educated specialists performing different kinds of nose surgeries — cosmetic and medical — Iran enjoys one of the richest manpower resources for nose surgery in the world.

What is rhinoplasty?

A rhinoplasty or nose surgery (also known as ‘nose job’) is a plastic surgery aimed at correcting, reshaping, or reconstructing the nose. The rhinoplasty procedure is typically performed to address the aesthetic aspects of the nose. It involves a surgical procedure to modify the structure of the nose, usually by making changes to the nasal bone or cartilage.

Not all rhinoplasty procedures, however, are for cosmetic reasons. Sometimes nose surgery is performed to restore the function of a nose with medical problems, e.g. difficulty in breathing because of a deviated septum. The surgical procedure for correction of a deviated septum is technically called septoplasty.

What can rhinoplasty fix?

Types of noses in terms of rhinoplasty

There are two main types of noses as far as rhinoplasty is concerned: bony nose and fleshy nose (meaty nose). The defining factor in differentiating between bony noses and fleshy noses is the thickness of the skin. Bony noses have thin skin or medium-thickness skin, but meaty noses have thick skin.

Because of the thinness of the skin in bony noses, the details of their structures are clearly recognizable. These noses generally have stronger cartilage, which leads to the nasal tip to be firmer with less likelihood of droopiness.

On the other hand, fleshy noses do not reveal the details of their bony structure and cartilage because of their thick skin. Furthermore, these noses have weaker and thinner cartilage tissues.

Sometimes, however, the above general rule does not apply, i.e. a bony nose might have think skin and a fleshy nose might have a thin skin with strong cartilages.

Bony nose surgery vs. fleshy nose surgery

Different techniques are used to do a rhinoplasty for fleshy noses and bony noses. A challenge in meaty nose surgery is the reshaping of the tip of the nose. In fleshy nose surgery, a good result is achievable by refining the sides of the nose and reshaping the nose tip.

However, because of the possibility of deformation of the lower part of fleshy noses after rhinoplasty, which is a result of their weak cartilages and heavy skin, new techniques are used to make a support for the nasal tip and prevent it from dropping.

In bony noses, small changes to the structure will bring remarkable results. Unlike meaty noses, bony noses do not have much fat under the skin. Swelling after surgery will be less severe in bony noses than fleshy noses.

For cases with a bony nose, the surgeon must perform the operation with utmost delicacy so that there would be no asymmetry in the final result. Choosing an experienced and skillful surgeon will be the most important step in achieving the desired result from a boney nose surgery or a meaty nose surgery.

Common Iranian nose job styles

It is very common to hear Iranian rhinoplasty surgeons and patients referring to different nose job styles as ‘fantasy‘ (or ‘dolly‘), ‘semi-fantasy‘, or ‘natural‘.

Natural nose job

In the ‘natural’ rhinoplasty style, smaller changes are applied to the nose, with the result being a nose with a straight nasal bridge rather than a curved one. Also, the angle between the nasal tip and the upper lip is not too wide in a ‘natural’ nose job style.

‘Natural’ style for bony noses

Bony noses with dorsal humps are good candidates for ‘natural’ style.

‘Natural’ style for fleshy noses

Fleshy noses usually receive ‘natural’ style rhinoplasty.

Fantasy nose job (dolly nose)

To put it simply, a ‘dolly‘ nose (fantasy nose) is a very small and upturned nose (informally known as ‘ski jump’ nose or ‘snub’ nose). In a ‘dolly nose’ surgery in Iran, the nose is made small and upturned (so-called ‘retroussé nose’). It produces a favorable result mostly in bony noses and is suitable for and most attractive in small faces.

Semi-fantasy nose job

A ‘semi-fantasy’ nose job style is something between the ‘natural’ style and the ‘fantasy’ style. The curve of the bridge is slighter and the angle between the nose and the lips is narrower than a fantasy nose.

Fantasy and semi-fantasy noses are more popular among women than men, who typically opt for natural style. The desired result in these two styles greatly depends on the skill and experience of the surgeon, as well as the type of nose.

Nose job for men (Male Rhinoplasty)

Rhinoplasty or nose job is not only for women. An increasing number of men undergo rhinoplasty every year to change the properties of their noses and achieve a better-looking appearance. Even though this cosmetic surgery is becoming more popular to both genders and increasing number of male celebrities are undergoing nose job, but male-rhinoplasty is not exactly the same as female rhinoplasty.

Although plastic surgeons use the same surgical techniques while performing the surgery, the results of male rhinoplasty surgeries are different from the results of women nose jobs due to differences in anatomy and preferences.

Men’s noses are different from women’s. They tend to have straighter and wider bridge, have thicker skin, and their tips is often more bulbous and broader than women’s noses. How long should I wait after a previous surgery to undergo a revision rhinoplasty?

You should wait at least 10-12 months after your previous rhinoplasty before considering a revision rhinoplasty when your nose has taken its final shape. It takes at least one year for the swelling to completely subside.

20 days after my nose job and i still have a lot of swelling. Is it even normal? And how long is it gonna last?

Experiencing swelling 20 days after rhinoplasty is not strange.
While the majority of swelling subsides in 3 weeks and patients will feel quite comfortable being in public, it won’t completely vanish so soon.
Rhinoplasty recovery varies from one person to another and depends on different factors such as the thickness of the skin, how complex the procedure has been and if it involved fracturing bones, the method, grafting material, number of rhinoplasty times, etc.
Your nose swelling gradually diminishes usually until a year after your surgery. You can expect your nose to settle into its new shape in about 6 to 12 months, in some cases, it takes even longer.

How serious is this quitting alcohol before rhinoplasty? My doctor said i need to stay away from alcohol two weeks before the surgery and i want to know how fixed this number is and if drinking before rhinoplasty is really that important.

That was actually a very important question, thanks for asking.
And yes, it is a serious piece of advice you need to follow both before and after your rhinoplasty.
Alcohol can thin your blood and interfere with blood clotting. Therefore, not only does it make controlling blood loss during the surgery very difficult, but it can also come with an increased risk of post-operation bleeding which is a serious complication.
Moreover, alcohol interferes with absorbing anesthesia and may lead to using the wrong doses which can be dangerous.
The increased risk of infection during and after surgery is another danger of drinking alcohol.
A period of two weeks of avoiding alcohol (before and after surgery) is what most surgeons agree on, and that’s what we ask our patients to do.
And it is very important to stick to this rule for your own safety. Generally, the longer you quit alcohol the smoother rhinoplasty and recovery you’ll experience.

Up to now, no method has been developed to reduce nose size without surgery. While non-surgical rhinoplasties, also known as liquid or injectable rhinoplasty have been developed to help with creating a more even and smoother nose by hiding mild dorsal humps, filling the depressed areas, and improving nasal tips, the liquid injection can not decrease the size, and in most cases, it may even make the nose look larger by adding fillers to straighten the nose.
It is also important to note that non-surgical methods have temporary effects and must be repeated every 6 to 12 months to maintain the result.

I am 33 years old and the bump on my nasal bridge has always been on my nerve. Yet the idea of breaking the bone during nose job really freaks me out which had me hesitating so far. Is it possible to remove the dorsal hump without having to break the nose?

It may be possible. But let’s talk about this “Breaking the nose” first.
Maybe all the fears come down to this term, which does not properly describes what happens when a surgeon tries to remove a dorsal hump. Removing the humps and straightening the bridge is one of the most common applications of nose jobs. Yet, it is performed through specialized surgical instruments to create some fine cutes and help move the bone inward in a perfectly controlled way which can not be referred to as breaking. While repositioning may come with more bruising under the eye, it does not significantly increases the pain. Still, modern techniques have been developed to minimize bruising and swelling. A small percentage of dorsal humps can be removed without such fractures, usually minor ones. Still, a case with the indication that requires osteotomy(cutting bones) will not benefit much from rhinoplasty if bone repositioning is avoided. Therefore, I recommend a personal visit with your trusted surgeon for a thorough consultation to discuss your expectations and see if you can achieve your goals without undergoing osteotomies.
The bottom line: If you decide to get rid of the dorsal hump and your surgeon needs osteotomy to reshape the nose, you will definitely find it more helpful to let it happen as it’s the only way to bring about desired changes. Consider that you will not feel what happens during the surgery and it is performed just like other rhinoplasty maneuvers. Also, you will not experience more pain after the surgery. So, forget about “breaking the bone” and think about the benefits. Undergoing rhinoplasty without getting desired results would be much more disappointing, making you wonder why you went under the knife in the first place.

I had nose surgery mainly to remove my dorsal hump, more than a week ago. I just had the splint off and can’t believe what i see. My nose is still bumpy. Almost no change has happened. It even seems bigger. I have read all about the swelling and the stuff, but can swelling also cause a nose to look bumpy or crooked? Is there a chance that it will look better over time? Please tell me i haven’t spent thousands of dollars to get a week off with bruised eyes.

Patience dear. One week or so is too soon to come to a conclusion.
There’s still substantial swelling and yes it is common that uneven swelling gives the impression of a persisting hump. 
The skin needs much more time to settle and it can take even months for the nose to reveal its new look.
Your surgeon has most probably talked to you about the rhinoplasty recovery and how your nose may look over time. Still, you can discuss your concerns in the follow-up sessions.
Meanwhile, considering you have just taken off the splint, taping your nose for another week and patience can help a lot.
Note that while you may only detect your nose swelling for about three months, swelling will persist for about a year or even longer and that’s when you can judge your final rhinoplasty outcome.

I had my nose job 16 days ago and i guess i had a good surgery. I am following the post op instructions pretty well. Except for yesterday when my friend who is quite a clown, came to visit and no matter how hard i tried i could not help but laugh out loud a few times and now I am really nervous if i have ruined my rhinoplasty results. How badly is it gonna affect my nose?

I’m glad you had a successful procedure and it’s really nice seeing patients take good care of themselves after their rhinoplasty. But please don’t worry about laughing at this stage of your recovery. It seems that you’re not having any odd symptoms to mention and it’s almost unlikely that laughing damage your nose job results. So I guess it’s safe to say there’s nothing to worry about. 
While extreme facial expressions in the early days following your rhinoplasty can put a strain on your operated nose and cause the incisions to open up, it is often safe for most patients to laugh and smile two weeks after their surgery. However, you should know that each patient has a different healing process, so in case you’re experiencing pain, tenderness, bleeding, or any other unusual symptoms, make sure you visit your surgeon for an examination.

I really think I can use a rhinoplasty and it has been a while since I am considering a nose job. But I am worried about sneezing after nose job. I have heard it is dangerous to sneeze after rhinoplasty and considering my allergic rhinitis, it happens a lot that I need to sneeze and I cant help it. What can I do?

Yes, it is actually something to consider but there are ways to manage the problem and minimize the risks.  
Nose blowing and sneezing within the first few weeks after rhinoplasty can increase swelling or cause bruising and bleeding. In very extreme cases, it can even contribute to moving the nose cartilage or bone and damage your nose job results. That’s why surgeons often advise you to be very gentle on your nose in the first stage of your recovery (mainly the first two weeks or more) and avoid coughing, sneezing, and blowing the nose as much as possible.
 As someone with allergic rhinitis, you must discuss your condition with your surgeon. The first thing you should do is to schedule your surgery for a season when you have fewer symptoms. Your surgeon will probably prescribe you medications to reduce the allergic symptoms after rhinoplasty.  
It is understandable that while you can decrease the chance of sneezing by staying away from allergens and taking medications, it is not always possible to completely avoid sneezing. So we advise the patients to sneeze with their mouth open to reduce the pressure on their new nose and prevent the consequences. Also, control yourself not to blow your nose too hard in the first few weeks. These precautions often help patients, even those with allergic rhinitis, have a smooth recovery without complications.

I am 10 days into my rhinoplasty and I have been crying since I got my splint off. I have got a piggy nose and I hate it. My friends are gonna laugh at me. I feel it is getting worse each day. Can I have it fixed without a revision rhinoplasty? And if not, how soon can I have another rhinoplasty?

Dear, I understand your concern but you really need to stop crying and be patient. But before I answer your question I would like to add a note in parentheses: The first thing you should notice is that crying after rhinoplasty, especially during the first two weeks, can worsen swelling and cause bruising, exactly what you want to decrease as much as possible in your recovery.
Now, let’s deal with the reason for your disappointment. This is too early to judge your result after 10 days cause you’re still experiencing significant swelling in the base of the columella, upper lip, and nasal tip which can contribute to creating the illusion of a too upturned nose. As the swelling subsides, you’ll start to get a more realistic idea about your nose shape and it can take up to a year to see the final outcome. While major swelling resolves in three months, we still ask patients to wait for a whole year before considering a revision procedure or any kind of intervention. Your concerns may be fully resolved by then and you may no longer require a secondary procedure. But if you still want a change in one year, I’d recommend a revision rhinoplasty with a board-certified surgeon with expertise in revision cases. While there are non-surgical methods such as filler injections, the changes are minimal, temporary, and usually not very satisfactory.
Well, now, let’s get back to crying. Although it can’t physically damage your results, it can give rise to swelling which in turn could be responsible for an over-rotated look of your nose. So, what you can do now is to follow the instructions for a smooth recovery and minimize the swelling.

I am 19 years old and apparently I have got a deviated septum which kind of disrupts my breathing but I can live with it. My doctor said a septoplasty could fix my breathing problem. I am now wondering if my nose shape will also change and can look better after this surgery as I have got a droopy nose with a small hump and having it fixed makes the surgery worth it. I mean is septoplasty the same thing or does it do the same thing as rhinoplasty? Cause I can name at least two of my friends who deny having a nose job claiming that it had been a simple septoplasty to get rid of their polyps and breathing issues but came out a whole different person with a very nice nose from the surgery.

Thanks for your question. That’s what many patients ask and I’m happy to clarify a little. A septoplasty is a surgery dealing with functional issues and the inside structure of the nose such as fixing a deviated septum and breathing difficulty, while a rhinoplasty deals with the aesthetic aspects of the nose and what is seen from the outside. Therefore, having a septoplasty will not help enhance a droopy nose, straighten or create a curve on the bridge, and other aesthetic changes. However, a septoplasty and a rhinoplasty can simply be combined (known as septorhinoplasty) and be performed in one surgical session by a single surgeon experienced at both procedures, to bring about the benefits of both; functional and aesthetic enhancements. In fact, they’re often combined and many people having a nose job may have undergone a septorhinoplasty instead of a simple septoplasty or rhinoplasty. Yet, some patients would love to hide having cosmetic surgery, especially when the improvements are minor. But it can’t change the fact that they have undergone structural changes to bring about aesthetic enhancements. So, if you also feel the need for a change in your nose shape and are afraid of having two different surgeries, a septorhinoplasty is what you need. But you need to notice that the cost could be different.
